Coordinated Family Support Center adds Spanish-Speaking Coordinator

The Coordinated Family Support (CFS) project at the Center for Persons with Disabilities has received a grant from the Utah Department of Health enabling the addition of a Spanish-speaking care coordinator.

 

More than 100 families who have children with special health care needs currently receive resource information, assistance and support in accessing programs and services that they have identified as important to their child and family.

 

The CFS project partners with various medical agencies and organizations to provide a medical home for families and their children with special health care needs in a three-state area, primarily in rural areas. The CFS project works with the local Hispanic Health Coalition to better meet the needs of CSHCN for culturally diverse families.
